Pillbox Hospital MLO for FiveM
Pillbox Hill Medical Center is where most EMS roleplay on a FiveM server ends up. Shootouts, crashes, overdoses, and bad decisions at 3 a.m. all route back to the same building, so the hospital interior you pick gets more foot traffic than almost any other map you install. This page walks through what to look for in a Pillbox hospital MLO, which Pillbox and hospital options we carry at FiveM MLOs, and how to get one running without breaking your existing map stack.
Why Pillbox Is the Default EMS Hub
Pillbox sits in the middle of downtown Los Santos, close to Mission Row, Legion Square, and most of the city’s busiest streets. Players already know where it is, ambulances reach it fast, and police can hand off patients without a long drive. That’s why most servers treat it as their main hospital.
The problem is that the vanilla building doesn’t give EMS much to work with. A proper MLO turns it into a working space: a front desk for check-in, treatment areas for the ER rush, rooms for surgery scenes, and places where doctors can actually sit down and do paperwork. When the interior supports the flow of a shift, your EMS department stops improvising and starts running real scenes.
What to Check Before You Buy a Pillbox MLO
Every Pillbox interior is laid out differently, and the gallery is your best source of truth. Before checkout, go through the screenshots and the listing notes with your EMS leads and confirm the things your department actually uses.
Layout and rooms
- Reception and waiting area, so civilians have somewhere to go that isn’t the trauma bay
- Emergency or triage rooms close to the ambulance entrance
- Operating rooms or surgery spaces if your server runs surgery RP
- Patient wards or recovery beds for longer scenes
- Offices, a break room, or a locker area for staff
- Any extras shown in the gallery, such as a helipad, garage, or morgue
If a room isn’t shown in the gallery or mentioned in the listing, assume it isn’t there.
Exterior, parking, and access
Think about how ambulances arrive. Check where vehicles can park, whether there’s a covered drop-off or garage, and how far a stretcher has to travel to reach a bed. A long walk from the ambulance bay slows every call.
Performance on busy nights
Hospitals fill up during big events. Look at how detailed the interior is and test it on a staging server with a realistic player count before launch. If your server already runs a lot of downtown MLOs, the 2026 FiveM MLO priority stack is a good way to decide what earns a spot and what can wait.
Pillbox and Hospital MLOs at FiveM MLOs
We carry a few Pillbox-focused options plus other hospital interiors for servers that want more than one medical location. Open each product page for the current gallery and description.
- FiveM Pillbox MLO: a Pillbox Hill Medical Center interior. The listing describes emergency rooms, operating theaters, patient wards, and administrative offices, with reception desks and waiting areas.
- Gabz Pillbox Hospital Red: the listing describes 30 rooms, more than 1,600 furnished objects, and an update that added parking lots, a garage, and an extra lobby.
- Billboard Pillbox Medical Center: not a hospital interior. It adds display billboards around the Pillbox area for EMS notices or server branding, and it’s meant to run alongside whichever Pillbox MLO you pick.
If you want a second hospital for another part of the map, look at the Paleto Hospital MLO for north county coverage, or compare Pillbox with our Ocean Hospital MLO guide. You can see every medical interior we stock in the FiveM hospital MLO category.
Avoiding Conflicts With Vanilla Pillbox and Other Maps
Most Pillbox problems after install aren’t bugs in the map. They’re two resources trying to use the same space. Before you go live, check a few things:
- Whether the MLO replaces the default hospital or is built as an add-on next to it
- Whether you already run another Pillbox or downtown hospital resource
- Whether an IPL loader such as bob74_ipl, or a map pack you installed earlier, touches the same area
- Whether nearby downtown MLOs overlap the hospital footprint or its parking
Run only one Pillbox interior at a time. If you’re swapping hospitals, remove the old one completely instead of stacking them and hoping the newer one wins. Our guide to FiveM MLO IPL conflicts covers the usual symptoms, like flickering walls, missing floors, and invisible doors, and how to track down which resource is causing them.
Setting Up Your EMS Job After Install
A map is just a map. It doesn’t come with an ambulance job, a billing system, or bed logic, so your existing EMS scripts keep doing that work. What changes is where those scripts point. After you install a new Pillbox interior, plan to update:
- Duty and clock-in points
- Bed, check-in, and respawn coordinates
- Stash, pharmacy, and armory or locker locations
- Garage and vehicle spawn points for ambulances
- Doorlock entries and target zones for restricted rooms
Walk the building with an EMS supervisor on your test server and note every interaction point before you edit configs.
Quick Install Steps
The basic process is the same for most MLOs. Drop the resource into your server’s resources folder, add its ensure line to server.cfg, restart a test server, and then walk the whole building. Check collisions, doors, stairs, lighting, and every spot your EMS scripts depend on. For the full walkthrough, see how to install a FiveM MLO.
When you’re happy with the hospital, it’s worth looking at the other side of most EMS calls. The Mission Row PD MLO guide covers the police station that usually sits a few blocks away.
FAQ
Does a Pillbox hospital MLO only work with one framework?
The MLO itself is a map, so it doesn’t run ESX, QBCore, or any other framework logic. Your ambulance job and related scripts handle that. You’ll just need to update their coordinates to match the new interior.
Can I run two Pillbox MLOs at the same time?
No. Two interiors built for the same building will overlap and cause clipping, missing walls, or broken doors. Pick one Pillbox interior and remove the other.
Will a Pillbox MLO conflict with bob74_ipl?
It can, if your IPL setup or another map also changes the hospital area. Test on a staging server and use the IPL conflicts guide to find the overlap before you go live.
Do I need a separate EMS script?
Yes. The MLO gives your medics a building. Revives, billing, beds, and duty status still come from your EMS job resource.
Which Pillbox MLO should I choose?
Start with what your EMS team actually uses on shift. Compare the galleries and listing notes for the rooms, parking, and extras you need, and pick the one that matches your department rather than the biggest building available.
